Make contracts and guidelines for volunteers

At this moment, there are two volunteers in Kisozi. In the future, we would like to see more volunteers get involved and moreover, we would like to be more professional as far as volunteers, attracting volunteers and guiding volunteers is concerned.
This means that would like to set up a database of volunteers that have worked or are working for HAU. Also, we feel that we need to make guidelines for volunteers. And we would like to make a contract for volunteers that work for HAU.
We would like to ask someone to help us with:
1) Designing a form that we can use to fill in basic information about our volunteers that we can keep in a file (such as name, address, work experience etc.)
2) Designing infomation/guidelines for volunteers that want to work for HAU (a sort of booklet). For more information contact Eddy or Esther.
3) Designing a contract for volunteers. This contract will include their tasks, the period that they will volunteer, rules that apply to the volunteers and rules that apply to HAU etc. Perhaps this could even include a privacy declaration. For more information contact Eddy or Esther.
We would like to finish this task by the end of September (or earlier when possible of course), so that we can set up this database in Kisozi for real in October.

I used one of the forms that can be found when clicking on the link that Claudia gave, as a model for an application form to volunteer with HAU. I have enclosed it here.

I know of some lady she is working with FSD and they allocate international volunteers here in Uganda and also a friend who is chairman AISEC Makerere University Business School Iwill try to talk to them. Mean while we can start in your country and the local Universities in Uganda and give them a Volunteering chance you know like those from your country come to Uganda as Volunteers but later End up Touring Uganda Easily and cheaply so they dont come as tourists but as voluteers or visitors of Hope alive Uganda.
As far as the Database Do you a computer based Database
Iam going to do some Googling for the guidlines and But mean while It needs people from HAU to set the rules since its them that know the place well. i believe that Rules and regulations depend on a place though there are comon ones

Hi, I am attaching a form which can be used for basic volunteer informations. At present it is in "word" but I can change it into "excel spreadsheet" if it is more convenient. The heading can be changed/moved around, omitting the running numbers etc. I dont know if this is what you are looking for. Kind regards
